Step 01

Register with Accord​

Chief pharmacist, or their appointed deputy, must first register with Accord as part of a controlled distribution system by completing and returning the Pharmacy Registration Form (PRF).

Step 02

Healthcare Professional brochure​​​

All HCPs must read and understand the brochure before prescribing or dispensing to any patient. Accord Risk Minimisation Materials can be found in the resources section

Step 03

Controlled distribution​​​​​

Once registered with Accord, thalidomide and/or lenalidomide can be obtained from our nominated controlled distributor to hospitals.

Step 04

Prescription Authorisation Forms

A PAF must accompany every prescription, and be sent to Accord at the point of dispensing to monitor the effectiveness of the PPP. Once registered, stock can be ordered by pharmacy prior to the completion and submission of a PAF.

Auditing requirements are the responsibility of Accord.
